PRESCRIPTION RENEWALS
If you require a renewal of your regular prescription(s), we would kindly ask that you have the pharmacy fax a renewal request to the office (fax: 1-844-321-8485). This is the most efficient way for us to address your request in the timely manner.
If this is a new prescription request, you would be required to make an appointment with your doctor.
